INformation for scheduling

Please enter your legal name, email, and cell phone number so we can contact you if there's a conflict with your appointment request. 

Provide details about your VISION PLAN, which is usually different from your medical insurance. All plans will be verified before approving your request. 

Appointment requests must be approved by our staff before being added to the schedule. If we need more information, we'll contact you via email or text. If you don't respond within 2 hours, your request will be canceled.  Same day requests must by made via phone call. Same day online requests WILL NOT be approved.

If you need special assistance (e.g., non-English speaking, wheelchair access, hearing difficulty), please do not use the online scheduler. Instead, call 610-913-0126.

 We may reschedule your appointment if it could cause a delay in our schedule.
