 Base Cost of Program:  $165 annually PER PERSON
INCLUDES
EXAM:
One (1) comprehensive vision wellness exam per 12 months with a $0 co-pay (normal cost $125) Does NOT include medical visits such as Diabetic exams and Glaucoma exams.
Two prescription rechecks within a 12 month time frame with a $10 co-pay each visit (normal cost $40 each)
Contact Lens Evaluation: Discount $20 (normal price range $75-150) and includes up to 60 days of follow-up care.
Please note medical eye care (red eyes, glaucoma checks, injury care, etc.) is NOT covered under this plan and will be billed to your medical plan.  We are required to collect appropriate co-pays at the time of your visit as contracted with your medical plan.
